This will allow your assigned mortgage counselor to review your information and see if you qualify for home mortgage modification.
Your assigned counselor will also be able to provide you with a login ID allowing you to check the status of your application 24 hours a day. If you have already applied and have not received a login ID click here to request one.

In addition to the online application, your lender may request the following items:

 

-  2 months paystubs

-  2 months bank statements

-  Award letter(s), if applicable

-  Most recent W-2(s)

-  Proof of Homeowner’s insurance policy (declaration page)

-  Mortgage statement(s)

-  Supporting hardship information, if available

-  Profit & Loss statement and most recent tax return, if self-employed
